    Key findings

    • The spread is real money. At 65, Plan G runs from $2,245 a year (about $187 a month) at the low end to $4,517 a year (about $376 a month) at the high end. That is $2,272 a year for benefits that are identical by law.
    • The gap grows with age. At 75, the same comparison runs from $2,999 to $6,499 a year, a $3,500 annual difference, and the ratio between the most and least expensive carrier widens from 2.0 to 2.2.
    • High Deductible Plan G is the quiet bargain of the table. Eight carriers offer it in Los Angeles, from $525 a year at 65, about $44 a month, in exchange for a higher annual deductible before the plan pays.
    • Plan N runs from $1,704 to $3,331 a year at 65, tracking the same two to one spread as Plan G, with small copays and possible Part B excess charges as the tradeoff for the lower premium.
    • 18 of the 19 carriers use attained age rating, meaning the premium climbs as you get older on top of any rate increases. Only Transamerica Life uses issue age rating in this market, which prices higher at purchase but does not add age based increases afterward.
    • California's birthday rule makes this table actionable. Every year, in a window starting on your birthday, you can switch to another carrier's plan with equal or lesser benefits with no health questions, so a Plan G holder paying near the top of this table is never locked in.

    Methodology

We retrieved these premiums from the California Department of Insurance 2026 Guide On Medicare Supplement Insurance, the regulator's own comparison tool, on September 1, 2026, for Los Angeles County, individual policies, non tobacco rates, at ages 65, 70, and 75. Premiums shown are annual amounts as filed with and approved by the regulator; monthly figures quoted in the findings are those amounts divided by twelve. Carriers appear under their legal filing names. Rates can change through carrier rate actions and differ by county, age, tobacco status, and household discounts, so treat this table as the map of the market rather than a quote. We are an independent agency and do not sell every carrier listed; the table includes every carrier the CDI tool lists for this county regardless of whether we represent them.

    Questions about these rates

Because the benefits are standardized but the prices are not. California requires every Plan G to pay exactly the same benefits, so carriers compete on premium, on their history of rate increases, and on underwriting, not on coverage. A carrier's price reflects its claims experience, how it rates by age, and its market strategy: some price low to grow, others price high and rely on customers not comparing. That is how the same plan spans $2,245 to $4,517 a year at 65 in Los Angeles. The practical conclusion runs in one direction: never buy a letter plan without seeing the whole table, and never assume a famous brand prices competitively. The benefits clause in your policy will read the same either way; only the premium and the company's rate increase history differ.

Attained age rating means your premium is based on your current age, so it climbs as you get older, on top of any general rate increases the carrier files. Issue age rating means the premium is based on your age when you first bought the policy; it still rises with carrier rate actions, but not simply because you aged. In this Los Angeles table, 18 of the 19 carriers use attained age rating and one, Transamerica Life, uses issue age. An issue age policy often costs more at 65 but can cross below an attained age policy years later. When we compare plans we look at both the price today and the carrier's rate structure and increase history, because the cheapest premium at 65 is not always the cheapest premium at 80.

For many healthy budgets it is the best value on the table. High Deductible Plan G gives you the same benefits as standard Plan G after you meet an annual deductible, and in exchange the premium drops dramatically: in Los Angeles it starts at $525 a year at 65, about $44 a month, compared with $2,245 for the cheapest standard Plan G. If you rarely use care, the premium savings can exceed the deductible you would pay in a bad year, and the plan still caps your exposure. If you see doctors constantly or want first dollar simplicity, standard Plan G or Plan N fits better. Eight carriers offer the high deductible version here, and the right answer depends on your health, your cash cushion, and how you feel about paying a deductible in a rough year.

In California, yes, once a year, without health questions. The state's birthday rule gives every Medigap policyholder an annual window, beginning on your birthday, to switch to another carrier's plan with equal or lesser benefits with no medical underwriting. If you hold Plan G with a carrier near the top of this table, you can move to a cheaper carrier's identical Plan G during that window and the new company cannot decline you or price you on your health. The savings compound, because you can repeat the exercise in future years as carriers change their rates. We run this comparison for Supplement clients every year before their birthday at no cost. Call (818) 472-5484 and we will tell you exactly what the move would save at your age.

They are the regulator approved annual premiums for individual non tobacco policies in Los Angeles County at three sample ages, which makes them the right map and an imperfect quote. Your exact premium depends on your age at enrollment, tobacco status, available household discounts, and any rate actions a carrier has filed since this data was retrieved. Some carriers also apply underwriting outside guaranteed issue windows, which affects whether you can switch at all. Use the table to see the shape of the market: how wide the spread is, where each brand sits, and how prices climb by age. Then get exact quotes for your birth date before deciding. A licensed independent agent can pull current figures across many of these carriers in one conversation, at no cost, and your premium is the same whether you buy through an agent or directly.
